I will be happy to provide you with documentation needed to file an "out-of-network" provider claim with your insurance company. Please check with your insurance company to ensure they reimburse for out-of-network providers. 

Note: I require a minimum of 24 hours notice to cancel an appointment. Cancellations occurring within 24 hours will result in payment of the full session rate.

Clients may request copies of their health care records by submitting a written request directly to this practice. Requests may be made via secure email or in writing. Records will be provided in accordance with applicable state and federal laws.

Filing a Consumer Complaint
